Award Reporting

If your organization is awarded a grant, you will be asked to sign a donor agreement which states that the funds will be used and reported on within a year.  The Survey Monkey Apply Form does allow for reporting but the current preferred way is to send a direct email to Lisa Meiling. While the reporting form is flexible, please include the following information:

  1. Organization name, contact information

  2. Amount and date of award

  3. How the funds were used

  4. Alignment of the intent of the original application and the outcome

  5. Impact of the grant

  6. Additional information as suited to the project i.e. narrative examples, case stories, photos, statistics
